The initial disclosure or exemption attestation is followed by a recurring statement. It is distinct from the first filing and should have its own calendar entry.

What the official record says

Subdivision (g) names four categories: beneficial ownership disclosure information, the street address of the principal executive office, exempt-company status if applicable, and other information designated by the Department of State. Subdivision (b) separately says an exempt company is subject to this annual statement requirement in the form prescribed by the department and must attest under penalty of perjury. The statute does not turn the annual statement into a public owner directory; subdivision (f) makes natural-person beneficial-owner information confidential subject to listed exceptions.
